Register for the plan of your choice. When registering, you’ll need your:

• Online Banking Information or Routing and Account numbers.
• Valid Email and Phone Number.
• Driver’s License Number.
• Social Security Number.

DirectVision applies all processing and handling to each patient account which is based upon volume of members on the account. Processing and handling covers billing verifications, communications, support, and account management associated with your account. To enroll in the DirectVision network, you must be 18 years or older. This is an auto-renewal contract, with three notices given prior to renewal. Your benefits are provided through the eye care provider you enrolled with, and all benefits and terms of use are determined by their specific policies, whether they are open-ended or single-use. Open-ended benefits can be utilitzed as many times as needed while you are an active member of your provider’s vision plan, whereas a single-use benefit can only be used once per person enrolled under your membership agreement.
